Montana First Nation is seeking a highly motivated individual to work as the MISD Coach and will report directly to the Integrated Human Services Manager of the Montana Integrated Service Delivery (MISD) and is responsible for assisting the MISD team in the service and delivery of programming, ensuring control and accountability while maintaining strict compliance to Montana First Nation policies and procedures. Providing these services in an effective and efficient manner will ensure that the Department’s Programs are in the best interest of the Montana First Nation members and their families.

Duties and Responsibilities:

Will be required to learn, understand and implement the integrated service delivery model and deliver services as per the MISD Process

• Assist with the collection of necessary data to assist in identifying members needs and identifying social determinants of health

• Build and maintain relationships with members which includes monthly interaction, and may include home visits, to improve MISD’s ability to identify members needs

• Assisting members in creating pathways to self sufficiency and healthy lifestyles by helping members develop important skills and strategies for personal growth

• Assist members in taking necessary steps to achieve their goals

• Collaborating with the MISD team to ensure delivery of programs and services are delivered in adherence with program policies and procedures

• Monthly updating and documenting of member information, member interaction, member progress in the database system i.e. 360 software, excel spreadsheets etc., to ensure information is accurate and up to date

• Ensure the confidentiality and security of all member information

• Identify all possible internal and external resources to enhance MISD service delivery to ensure members have access to services

• Assist in the delivery of programs and services to members, which may include activities after hours

• May be required to conduct home visits to identify needs of members

• Support the proper procedures as it relates to MISD manuals to deliver programs and services

• Maintain regular consistent and professional attendance, punctuality, personal appearance, and adherence to relevant health and safety procedures

• Monthly reporting to MISD Integrated Health & Human Services Managers

• Any other related duties as assigned by the MISD Integrated Health & Human Services Managers

Qualifications and Requirements:

1. College or other post-secondary training in the Social Science field or Humanities are preferred

2. Must have a minimum of 2 (two) to 4 (four) years’ experience in the field of Social Science or Humanities, and must possess Secondary School Diploma or GED Equivalent

3. Reliable transportation and a valid Driver’s License required

4. Excellent interpersonal and strong communication skills required

5. Experience in a comparable work environment with similar scope, independence, responsibility and authority an asset

6. Strong organizational skills required with the ability to work under pressure, meet tight deadlines and handle competing priorities; ability to multitask

7. Must be proficient with computers and Microsoft office suite, including keyboarding, word processing and email

8. Knowledge and understanding of the Montana First Nation reserve, membership, language, culture and traditions an asset

9. Must be drug and alcohol free for at least 3 years.

10. Must be willing to sign and Oath of Confidentiality and must abide with all MFN Policies and Procedures for Employees.
