GENERAL DESCRIPTION
Reporting to the Oskayahk Assisted Living Manager, the Oskayahk Worker primary responsibilities are provide initial supports and services to newly transitioning youth from care, ages 18 to 26 years old. This includes: implementing information & orientation workshops, assessing participant’s relevant strengths and needs, developing actions plans, conducting follow-up assessments, resolving conflict situations, liaising with other social service agencies and health care providers and maintaining program statistics for purpose of reporting and evaluation.
Service Responsibilities
Under the direction of the Oskayahk Manager, the Oskayahk Worker is responsible for facilitating and delivering effective transition from care services. The Oskayahk Worker is designed to support youth who are leaving government care and who may not have completed their transition to independence plan or additional supports to achieve their goals.
Program Highlights:
· Available to youth between the ages of 18 – 26.
· The amount provided and items covered are all determined in negotiations with the youth and caregivers. Room and board plus a small amount for personal items are not uncomment arrangements.
· Caseworkers have the ability to negotiate fees with caregivers if they are providing a service.
· Agreements are signed for a maximum of nine months, but in some regions three-month agreements are more common.
· Health Care provided (up to 20 years of age).
· Costs may be covered for programs associated with completing the transition to independence plan. If the youth will need to utilize the Oskayahk Program, it is important to discuss this with the caseworkers long before the youth turns 18. It is recommended that the youth start the process at a minimum of 17 ½ years of age, in order to have the process completed and the application signed and submitted on the youth’s 18th birthday.
· Conduct youths’ needs assessment focusing on transition from care services and refers youth to appropriate resources when needed.
· Develop and update individual orientation and transition plans with participant to address their changing and ongoing needs.
· Addresses the youths’ immediate and long-term concerns, researched and provides appropriate, accurate information.
· Provide guidance to meet immediate needs such as housing, education and cultural transition.
· Advocate on behalf of the youth, connecting them with relevant government departments and community organizations.
· Obtain youths’ written consent for the release of information to agencies on as needed basis.
· Develop, update and deliver workshops e.g., Life Skills, Health and Wellbeing, etc.
Education and Experience
· Degree or two-year post-secondary diploma in Social Work preferred. Equivalent experience may be considered under special circumstances.
· Minimum of two (2) years work experience in the area of child and family services.
· Must have a current & valid Alberta Class 5 driver’s license, own vehicle with $2M Auto liability insurance.
· Must have Delegation and CICIO training.
· Knowledge of the AMO Law, Bill C-92 and Louis Bull Constitution
· Knowledge of the Cree culture and language would be an asset.
· Strong ability to anticipate and address problems.
· Excellent time management and organizational skills with the ability to manage and meet multiple
deadlines, with a high level of attention to detail.
· Excellent communication skills and an ability to work collaboratively.
· Knowledge of and interest in non-profits and the communities they serve.
· Welcoming of diverse backgrounds, lifestyles, beliefs, and cultures.
• Must be able to work flexible hours.
• Must have a valid driver’s license and possess own vehicle.
• Must be willing to authorize a “Criminal Record Search” and sign an “Oath of
Confidentiality”.
