The Urban Office Transition Coordinator reports to the Manager of Community Wellness. The Transition Coordinator is responsible for advocating for Samson Cree Nation (SCN) members who are transitioning to the city of Calgary. The Transition Coordinator will act as a liaison to government organizations, social agencies, Indigenous organizations, and other supports and services that would benefit SCN members making the change in their lives.
Roles and Responsibilities
❖ Serve as a point of contact for SCN clients arriving in Calgary,
❖ Assist clients by providing information on programs and organizations that they may need,
❖ Meet with clients, by phone or in person, to determine needs,
❖ Provide referrals to required organizations; Calgary Food Bank, Sports Central, etc,
❖ Assist in logistics when required; i.e. SCN Christmas dinner, voting etc,
❖ Maintain a list of SCN members who are residing in Calgary and ensure information is flowing for engagement purposes,
❖ Assist clients have require access to public transportation,
❖ Be available for clients who are searching for information on services,
❖ Accompany clients, when possible, who do not have family support to appointments for supports and services,
❖ Any other required duties as assigned by Community Wellness, Nipisihkopahk Wellness and Social Development (NWSD) and SCN Administration.
Skills and Qualifications
● Must live in Calgary,
● Must have great communication skills, including writing skills,
● A minimum grade 12,
● Must have computer knowledge,
● Be knowledgeable in the organizations that provides support and services,
● Must be willing to travel.
